The Facial Plastic Surgery Center has made new patients forms and pre- and post-surgery instructions available online, providing easy access for our patients. All forms and instructions are available in a printable version.

Patients can complete the consent and medical history forms in advance of appointments and in the privacy of their homes. Please give the completed forms to the receptionist upon arrival.
